About Us
Family Care Certified Services is a Medicare Certified, Joint Commission Accredited (JCAHO) home care agency servicing Nassau, Queens. Family Care also has a Long Term Home Health Care Program (LTHHCP), which provides services to Medicaid clients in Nassau, Queens and Bronx Counties.
Through a Management agreement, Family Care has oversight of Datahr Home Health Care Inc. servicing Westchester, Putnam and Dutchess Counties.
Services provided include: Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Nutritional Therapy, and Home Health Aides. Family Care’s mission is to provide services that are quality managed, cost effective and culturally sensitive.
Family Care will extend the benefit of in home recovery to patients who require medical services normally associated with hospitalization.
Family Care will provide and coordinate the care that the Physician orders. We will explain the services to be provided that will assist the patient to reach and maintain optimum physical, mental and social well-being.
Family Care’s goal is to assist patients in getting better, regaining independence and becoming as self-sufficient as possible. |
