Family Care’s Long Term Program provides services that are quality managed, cost effective and culturally sensitive for patients in Nassau County, Queens County, and the Bronx. This program offers an alternative option for the chronically ill patient who would otherwise require placement in a skilled nursing home or health related facility.
Long term patients are chronically ill and require medical supervision to remain safely at home. Patients will receive in-home preventative diagnostic and supportive services. They will also receive intensive case management, access to community resources and entitlement counseling.
